	How do you get a free DAC report? Help!!!!!!!!!!!!!!!!!!!!!!!!!!

once there, you scroll down to "consumers".

then you click onto the link for the form.

it will come up, and you print it out

then you fill it out, and you must also have a photo copy of your drivers license AND SS card.

put it all into the mail, and its "about" 3 weeks waiting time.
